01:12 - After the night that I examined Grace,
01:15 so I went home to catch up some rest.
01:18 Around 23 hours, I heard a knock at my door.
01:28 Two women were knocking.
01:30 They said they brought a woman in labor.
01:33 So I thought maybe it was Grace,
01:34 not until they explained, no,
01:36 it's just another woman who's come for labor.
01:40 [gentle music]
01:42 After examining, that's when I saw
01:49 that this is a serious case.
01:51 The fetal heart was less than 100.
01:53 The baby was in distress.
01:56 So I had to call the ambulance,
02:02 though due to network failure,
02:05 the lines wouldn't go through.
02:09 The head was delivered,
02:11 and the shoulders couldn't rotate.
02:14 When the baby came out, it couldn't cry.
02:18 When the baby aspirated in conium,
02:20 there was no suction machine.
02:21 There was only a penguin sucker,
02:23 which couldn't go deep and remove those secretions.
02:27 I had to ambu-bag the baby,
02:30 but without good effect, the baby died.

04:35 My name is Peter Akswenya.
04:55 I am 55 years old now.
04:58 I'm a farmer.
05:00 I normally grow bays, groundnuts, green beans,
05:04 and some tomato.
05:06 I'm married to Grace Chibirika,
05:10 and I've stayed with my wife for now 16 years.
05:13 And I have four children with her.
05:17 The fifth is not yet born.
05:20 Yes.
05:21 [children laughing]

06:08 With my first wife, I've got five children.
06:11 They were supposed to be eight,
06:12 but the three died at an early age of their ages.
06:17 The first born daughter was memory.
06:22 She had malaria.
06:23 She died at the age of four years.
06:26 When I lost my second born,
06:30 I almost lost my life 'cause it pained me a lot.
06:34 Friends came to advise me to those other friends
06:38 that also lost their children.
06:40 Why should you lose your life
06:42 just because of this what has happened to you?
06:45 Then the third one, immediately she was delivered.
06:47 She only stayed maybe one hour, 30 minutes.
06:51 Then the infant died.
06:53 To experience death at that age,
06:59 you see your friend playing with his baby.
07:02 I only thought, why always on me
07:05 when my friends are keeping their children?
07:08 But I never took up my life.
07:12 I didn't lose my faith 'cause I know that death is there.
07:17 What I only did was to get more advices
07:22 how to keep my children,
07:23 and that's what I'm still doing now.

09:01 (speaking in foreign language)
09:13 (speaking in foreign language)
09:18 (speaking in foreign language)
09:22 - Yeah, for our clinic,
09:41 there are about four professional staff
09:43 working for, maybe attending to the population
09:46 of 10,061.
09:48 A clinic officer, midwife, and a nurse.
09:50 The other one is an EHT.
09:51 - In labor ward, we need a lot of instrument.
09:56 We need a resusterer in case the baby is born
09:59 with asphyxia, resustered a baby.
10:01 We need a suction machine.
10:03 We need catheters to empty the woman's bladder
10:05 when they're in labor.
10:07 We need sterilization machine,
10:09 but for here, Katova, we do not have those instruments.
10:14 This building, it has solar lighting,
10:17 so in the rain seasons,
10:19 we have to use our phones for lighting.
10:22 We learn to improvise,
10:24 'cause when there are no things,
10:25 at least you try to think
10:27 instead of letting a woman to suffer.
10:29 - This clinic is just for normal deliveries,
10:37 and a complicated delivery.
10:39 It has just to be referred.
10:43 If the nurse notices that something has gone wrong,
10:46 then the nurse will call the hospital
10:48 for an ambulance to go and pick the patient.
10:51 Okay, let me just call the ambulance then.
10:55 We can go.
10:55 (speaking in foreign language)
11:00 (siren blaring)
11:13 - Right now, we are responding to a call
11:15 at Kanaganchapa Rural Health Center,
11:17 which is about 10 kilometers away from the main hospital.
11:20 They are calling us for an antipaternum hemorrhage.
11:24 (dramatic music)
11:28 (speaking in foreign language)
11:31 - And they have to come to the hospital
11:48 where there's facilities for theater,
11:50 and so we'll then intervene by doing a caesarean section.
11:54 (dramatic music)
11:56 - So we have a big challenge,
12:03 because we have 39 facilities being serviced
12:06 by one ambulance, a population of close to 200,000 people,
12:11 and these facilities are not concentrated in one place.
12:16 To go to Katoba, which is one of our furthest facilities,
12:20 it takes about one hour, 30 minutes.
12:23 We have this huge challenge.
12:25 The nurse calls for an ambulance in Facility X,
12:28 which is 50 kilometers away, says this woman is bleeding,
12:31 and then we have another nurse calling from another facility
12:34 says, "I have another one who has failed to deliver."
12:37 So now, we are left with a dilemma,
12:39 because you have to decide,
12:40 where do you go to pick the patient?
12:42 (dramatic music)

19:54 I think this will be the last kid.
19:58 - Why do you think this is the last one?
20:00 - Are there enough that I have as at now?
