The accident happened at about 10h20 on Sunday, 02 August 2009 morning, during the second day of the Harry Flatters Rally, a round of the 2009 MSA British Historic Rally Championship. Spectator John Phillips who was watching the rally at Dixies Corner, near Sennybridge, Powys, Wales, was struck by the 1994 Peugeot 106 Type R driven by James Ball-Gavin Haycock. The out of control car went off the road and somersaulted being completely destroyed.

Mr Phillips, 69-year-old from Llandudno, Conwy, Wales, suffered serious injuries. He was airlifted to the University Hospital of Wales in Cardiff, where he later succumbed. The driver James Ball was taken to Prince Charles Hospital in Merthyr Tydfil with minor injuries.
