00:00As you know, RSPs, according to studies, a large population of women around the world experience pelvic inflammatory disease or PID.
00:12Every year, according to the World Health Organization, this is one of the leading causes of infertility or infertility in women in the reproductive age.
00:21But what's more alarming is that there are cases where this is not a symptom in the beginning.
00:26And today, Meiji, we will clarify this important question about pelvic inflammatory disease with Dr. Ferdis Galbo, an OB-GYN.
00:35Good morning, Doc.
00:38Good morning, good morning, good morning. Good morning to all RSPs. Good morning and thank you for inviting me here.
00:46Welcome, Doc. I love your energy.
00:49Today, this morning, this is Meiji together with Bien, Doc.
00:53Alright, Doc, what is pelvic inflammatory disease or PID?
01:00PID or pelvic inflammatory disease is an infection, a bacterial infection in the reproductive organs of a woman
01:07that is caused by STI or sexually transmitted infections.
01:11This is an issue that we need to discuss this morning.
01:17Doc, you mentioned that it is possible to get this from STI or STI.
01:24If it is not caught, what are the possible complications of this disease, Doc?
01:30Yes, this problem, as mentioned, is caused by sexually transmitted infections.
01:36Sometimes, their presenting symptoms are nonspecific.
01:41Meaning, sometimes, the woman does not feel anything.
01:43But usually, the most common presenting symptoms are vaginal discharge or pain.
01:49Unusual discharge that is sometimes smelly or itchy.
01:55Or sometimes, the different colors, like green, yellow, and so on.
01:59And also, the cause of pain in the reproductive organs of the woman, in the heart, in the matressed area.
02:07That is the problem with this.
02:10Doc, you mentioned the symptoms of PID.
02:14What if the woman does not feel anything?
02:17Is this still dangerous?
02:20Yes, it is dangerous.
02:21We call this a subclinical infection.
02:24So, as mentioned, sometimes, the presenting symptoms of PID are nonspecific or vague.
02:30That is why this problem is still caused by, sometimes, a bad problem.
02:35Inflammation, or inflammatory reaction, can cause the ovaries of the reproductive organs to stick together.
02:43That is why there is an obstruction.
02:46This obstruction can cause what we call an ectopic pregnancy.
02:51Because the sperm, egg, and fertilization cannot flow properly.
02:59That is why it can cause an ectopic pregnancy.
03:03Or sometimes, it can cause a grabbing infection in the matressed area that can cause sepsis.
03:08The problem with sepsis is that it spreads to the blood of the patient.
03:14That is why we need to give a solution immediately.
03:18The first solution is usually antibiotics.
03:23There are different classifications.
03:25It can be oral antibiotics.
03:27Or if we need to see an ultrasound of an abscess formation or tubo-ovarian abscess,
03:34we need to administer IV antibiotics.
03:38The problem with this is that we have what we call clinical signs and symptoms to assess if it will respond.
03:44If it does not respond, then we need to undergo surgery.
03:48The problem with surgery is that sometimes, what we call pelvic clean-up,
03:53the matressed area, the tubo-ovary,
03:56the source of the infection needs to be removed.
03:59That is the problem.
04:01Because it will cause the reproductive organs to be removed from the woman.
04:06Those are the worst scenarios.
04:08That is another one.
04:10I have to tell the patients, I have to tell our viewers, our RSPBs,
04:14STI is what you call chlamydia, gonorrhea.
04:18That is the most common.
04:20It is like a sister.
04:22But you have to think also, HPV,
04:24human papillomavirus that causes cervical cancer,
04:27is also STI.
04:30HIV,
04:32human infectious disease,
04:34or HIV, the cause of AIDS,
04:36that still can be transmitted and will present also as PID.
04:41Those are the worst scenarios that our RSPBs need to know.
04:45Doc, just a follow-up question.
04:48For example, if a patient, a woman, has gonorrhea, chlamydia, or HPV,
04:55does it need to be tested immediately for PID?
04:58Is it automatic that a woman will have PID?
05:02Is that how it is, Doc?
05:04Not always automatic,
05:06but it would be better to have a consultation immediately for testing.
05:10Then, our screening test,
05:12the test for pap smear,
05:14the gram staining, KOH, all those things,
05:16all symptoms, all results,
05:18to assess.
05:19Because if it is not tested immediately,
05:21it is very dangerous.
05:23As mentioned, it not only causes infertility,
05:25or ectopic pregnancy,
05:27it can cause also sepsis or infection in the woman's body.
05:32Oh my, this is really worrying, Meiji.
05:35Doc, what are the solutions that our countrymen can do
05:38to at least avoid PID?
05:42Okay, the solution is, first, prevention.
05:46That is really what we will talk about first.
05:48Prevention is that,
05:50we have to be frank with our viewers in CARASPP,
05:53especially our young population,
05:55the so-called party-party,
05:58or the one-night stands,
06:00we have to, as much as possible,
06:02refrain from it,
06:04because that is the source of sexually transmitted infection.
06:07If there is a need to have an abortion,
06:11then you have to protect yourself.
06:13You have to use condoms and all those protections
06:15to lessen the chances of STI
06:19that causes pelvic inflammatory disease.
06:22Doc, just a follow-up question,
06:24can PID be used again once our patient has recovered?
06:29Yes.
06:30There are factors that we call persistent or recurrent infection.
06:35First, you should not consult your doctor early.
06:38Second, you should not complete the prescribed medication.
06:43Sometimes, after two or three days, you say,
06:45oh, I'm okay, I don't need it anymore.
06:47No, you really need to complete the protocol
06:50to make sure that all the bacteria die,
06:53the cause of PID.
06:55Third, you have to treat the partner,
06:58the source of the infection.
07:00Because if you connect to your partner,
07:02the source of the infection,
07:04the source of the infection will be reversed
07:06and you don't know that it will get worse.
07:08So those are the three things that we really need to understand
07:13to prevent recurrence of infection.
07:16All right.
07:17Doc, ladies,
07:20Doc mentioned how we can avoid it.
07:23Yes.
07:24And of course, consult your doctor immediately.
07:26So once again, thank you so much, Doc,
07:29for joining us this morning.
