00:00 This week we're going to talk about do you continue to stay fertile after surgery for radical prostatectomy
00:07 So what does radical prostatectomy mean?
00:14 Radical prostatectomy just means that a removal of the whole prostate plus its accessory organ such as the
00:22 seminal vesicles
00:24 What does the prostate do? In fact, prostate produces the liquid for the sperm to swim in that is a
00:31 lubrication and also nutrition for the sperm. For when the prostate is removed there is literally no
00:38 ejaculation and therefore men will have dry orgasm, but there is no secretions at all
00:46 So what happened to all the sperms? In fact, the sperms continue to stay in the testicles and just
00:53 avoided being
00:55 ejaculated because there's no ejaculation
00:57 But for somebody who continue to want to father a child then there are two options
01:02 Before the operation you can preserve as much sperm as possible and freeze that for
01:09 rainy days in the future so that you can use the sperms
01:12 Alternatively in the event that you continue to want to have the operation despite having no preservation of
01:19 frozen sperm then we can do something called
01:23 PISA, which is
01:25 microscopic epidermal sperm
01:27 aspiration or PISA percutaneous epidermal sperm aspiration
01:33 Basically what it means is directly going to the access to the testicle and epidermis
01:38 Find the sperms and use the sperms for test tube baby or IVF
01:44 With such technology men don't even have to worry about not being able to father a child after treatment of radical prostatectomy
01:53 So there's no reason to avoid the operation for the cure of cancer
01:57 following radical prostatectomy
02:00 So for the following week, we want to discuss the problem about
02:03 hormonal suppressions and erectile dysfunction for men who use hormone for the treatment of prostate cancer
02:10 If you are on hormonal treatment and you're a bit curious about
02:14 How this is affecting you and they need to stay tuned for next week's putting Dr. G on the spot
