A pledget soaked in 0.5% proparacine is inserted into the fornix below the upper lid and kept for 2 minutes. Manual small incision cataract surgery is done with a 5mm superior sclerocorneal tunnel. A 6 mm ccc is done, followed by nucleus rotation and prolapse of the nucleus into the anterior chamber with a lens dialer. Nucleus removed over a Sheet's glide with pressure over the posterior lip of the wound . Irrigation and aspiration of the cortical remnants is done and a rigid PMMA lens introduced and dialed into the capsular bag. The conjunctval incision is co-opted with wet cautery . Patient walks out of the theater with a clear protective glass with immediate restoration of vision following manual small incision cataract surgery under enhanced topical anesthesia
