00:00Hey friends, I'm Stacey and this is Gorgeously Aging and today I'm going to do a little under
00:09eye refresh or under eye filler using Tesoro Deep With Lidocaine.
00:15I find this is the perfect viscosity for under the eyes and does a beautiful job.
00:19Today I'm going to use a 27 gauge 38 millimeter cannula.
00:24So I'm going to show you how to do a pilot hole, the pilot hole placement, and how to
00:28improve the under eyes.
00:29Now this is a cross-linked hyaluronic acid so caution needs to be taken.
00:34This is not medical advice or instruction.
00:37This is for educational purposes only.
00:39In the event of a vascular occlusion, hyaluronidase needs to be used to dissolve the filler.
00:44The use of a cannula makes a vascular occlusion very slim.
00:48The end of this cannula is not sharp so it doesn't penetrate into a vein or artery.
00:53It generally just navigates beyond them.
00:56Today I've sanitized my hands and put on some gloves.

01:13So I'm going to remove this topical lidocaine which has been on my skin for about 20 minutes.
01:18It can stay on longer than that, up to about 40 minutes.
01:23Prior to putting on topical lidocaine it is important to clean the skin.
01:27I use an alcohol swab so it's prepped and ready to go.
01:29Then place your topical lidocaine, allow that to sit for your preferred amount of time,
01:34and then I like to remove it with an alcohol swab.
01:37Now my primary concern today is my right eye and if I look down you can see I have a little
01:41bit of divot or hollow right there.
01:44I recently did an under eye treatment with Juvise which I think made my under eyes look
01:49really good.
01:50But sometimes there's a deeper hollow that isn't going to be treated with just a skin
01:55booster.
01:57So you're going to need the robustness of an under eye filler.
02:00Now not much is going to be needed here today.
02:02I'll show you just a little bit, we'll do the trick.
02:05And one thing I love about filler is that it is an instant improvement.
02:08I might just give a little bit of treatment to the tear troughs on both sides and just
02:13a quick talk about safety and anatomy.
02:15The angular artery runs down the nose here, the angular artery and vein, and it kind of
02:21comes down like this and then comes over.
02:24So the inside, where the tear trough is, is a little more risky than the lateral part
02:29of the under eye area.
02:31There is an infraorbital vein and artery kind of right in here.
02:34We're going to put our pilot hole just below that and if it bleeds a little that's totally
02:39okay.
02:40I'm going to make sure this is clean before we start the pilot hole.
02:45And before I do the pilot hole I'm going to take the cap off of the filler here and again
02:51This is Tesoro Deep with Lidocaine and I'm going to put the cannula right on here, just
02:56pop it on and twist.
03:00I'll leave this cap on until just the time that I'm ready to use it.
03:03And here is the pilot needle, this does have a sharp tip.
03:07And we're going to go in right about here and just make a little pilot hole.
03:20Because I like to leave it there.
03:21We're going to take the top off of the cannula and it helps to have a magnifying mirror.
03:31And we're just going to go right into that same spot and twist a little.
03:36We're going to advance down deep.
03:38We want to be not too close to the surface because that can be lumpy and bumpy.
03:42We want it to be a little deeper under the tissue.
03:46And sometimes twisting helps.
03:48I'm going to head over to the tear trough area here.
03:52And every so often you just pull up to see where the tip of your cannula is.
03:58You can also feel with your finger.
04:05Alright so I'm going to aspirate just a little bit.
04:09I'm not seeing any flashback in the syringe, which would be unlikely with a cannula.
04:17And as I put pressure on the syringe, I'm going to withdraw this here, slowly, and then
04:36without pulling all the way out, I'm going to turn the cannula and we're going to head
04:41up over to this primary area of concern, which is right here.
04:51And I'm feeling for the tip.
05:02And it's right where I want it to be.
05:04I'm going to pull back just to see if there's any flashback of blood right here.
05:09Again, that's very unlikely with a cannula treatment.
05:14And I'm going to go ahead and press the plunger.
05:20And then slowly withdraw.
05:31With this particular cannula, you do have to put a little pressure.
05:34Not an extreme amount of pressure, but you do have to put a little pressure.
05:37before you put the cannula into the eyelid hole.
06:07You can see that the cannula bends a little bit.
06:19Sometimes you can get some resistance and that's totally okay.
06:23If you do need to reposition it, don't use just your gloved hand, use a alcohol swab.
06:29Now as another option, I'm going to show you how to use a needle.
06:31So it does come with needles and I just screwed it on and took off the top.
06:35And right here where I have that deficit,
06:37it's filled in a little bit from that little bit of filler I just put with the cannula.
06:41But I think I want just a few more blebs there.
06:44So I'm going to go straight down until I hit bone, kind of like a cheek filler.
06:52And then I'm going to drop the angle just a little bit,
06:55pull back to see if there's any blood coming into the syringe.
07:00And then I'm going to go ahead and just deposit just a little bit,
07:06a little blob of filler there,
07:08massage that in right down to the periosteum, right down to that bone.
07:15And I'm going to do the same thing just below where I was.
07:20I'm going to feel the bone with the tip of the needle.
07:22I'm going to pull back. I don't see any blood there.
07:28Okay, now just a little bit of filler, just a little bit.
07:33And then pull out and massage.
07:37You'll feel little bumps there where the blobs of filler are. They will settle in.
07:42I'm going to do it here, down to the periosteum or the bone.
07:47Pull back. Watch for any blood filling into that syringe.
07:53And then deposit just a little tiny bit.
07:57Pull back.
07:59Now the disadvantage of this is you're going to maybe get a little bruising there,
08:02tiny little pokes on the surface of the skin.
08:05One thing I do like about a needle is it feels more precise.
08:08I can really get the filler exactly where I want it.
08:12And then I'm going to go ahead and just do the same thing on the other side.
08:15I can really get the filler exactly where I want it.
08:17I feel like there's a bit more control, but a cannula is definitely the safer option.
08:23Okay, I'm going to move to about right here.
08:27Going in.
08:32I'm not feeling much because that lidocaine that's in the filler
08:36has dispersed in that area from the cannula treatment.
08:41Okay, so we're going to aspirate.
08:44Pulling down.
08:45Be patient.
08:48No blood in the syringe.
08:51And I'm just going to go ahead and just give a little tiny bit here.
08:57And then we're going to massage.
08:59Now the reason you want that closer to the periosteum or bone as opposed to the surface
09:04is that one, it's safer, and two, you're going to get a smoother result
09:08than filler that's placed closer to the surface.
09:11And so far I've used half of this one ml of filler.
09:15So I've used about 0.5 ml.
09:17That's between the needle injection and the cannula.
09:20So I'm going to go over to this side here.
09:24Feeling the periosteum or the bone.
09:28I don't need as much on this side, so I'm going to be more conservative over here.
09:32I can feel that I'm hitting the bone there with the tip of the needle.
09:35It doesn't hurt.
09:36I'm going to pull back.
09:44And just give a little bit.
09:51Sometimes you can hear it going in.
09:52It might sound a little crunchy.
09:55It's just the sound of that filler kind of entering that space.
09:58And I don't want too much on this side because this side was definitely higher.
10:02And I don't want too much on this side because this side was definitely hollower.
10:07So let me just take a good look.
10:08I don't want to be overzealous.
10:12Okay, so to do the tear trough with the needle,
10:16it's a little bit more of a concern if you don't have experience
10:19because this artery is one that's important to miss.
10:23We're going to come in here.
10:29Okay, I'm going to go ahead and aspirate.
10:31I'm going to take my time with this aspiration.
10:39And as I pull back,
10:43I'm going to go ahead and
10:49and I'm going to just hold a little pressure on that.
10:54Kind of press it into that tear trough.
10:57Now, if you continue to hold pressure on the syringe,
11:01when you pull out the needle,
11:03you can get a little bump in the spot that the needle came out
11:06that will settle down with a little pressure and time.
11:09There's just a little inflammation there.
11:11So I like to turn my head to all directions to really look from all angles
11:16to see if there's any deficits or any areas that need just a little bit more.
11:22And sometimes that's hard to tell for a few days
11:24because there will be a little bit of inflammation.
11:37Remember, cannulas are always the safest.
11:39Less risk of bruising,
11:41but sometimes it is harder to navigate that cannula to the exact spot you want.
11:45So for precision, I like a needle, but there is more risk,
11:49more bruising, more little surface punctures.
11:52So there are definitely pros and cons to both.
11:55When doing filler, I always have hyaluronidase on hand.
11:59It's called LipoRace.
12:00That's something critical to have on hand if you are doing any kind of filler.

12:08So I'm just going to go ahead and just roll my finger along where that filler is
12:17on both sides, just a little roll back and forth.
12:21All right, there you have it, you guys.
12:22It's actually pretty easy the more that you get comfortable with it,
12:25but it's something definitely to study.
12:27And even though I've been doing this for years and I've been trained,
12:30I still do a quick refresh of facial anatomy before I start every single time.
12:36Knowing and feeling confident with your safety measures is definitely the key to success.
12:41Aspiration and having the right tools on hand,
12:44making sure you are sanitized and ready to go is also so important.
