00:00The challenges faced by people with immunodepressive diseases and infectious diseases have given us great satisfaction in recent years.
00:15I belong to a generation of lucky infectious diseases specialists who aim to eradicate Hepatitis C,
00:21treat Hepatitis B and, above all, have managed to make HIV a chronic disease with a series of treatments
00:27that improve the quality of life of these patients as well as their survival.
00:32These are encouraging data, those regarding innovative treatments for immunodepressed patients,
00:37emerged from the regional congress Simit-Campania, which took place in Naples.
00:41An opportunity to compare scientific updates on new immunological therapies against different pathologies,
00:46with a focus on new antiviral drugs for HIV patients.
00:50In Campania, the number of new cases remains stable, but the diagnoses are more and more advanced infections,
00:56hence the importance of screening, which allows to identify the virus in the initial phase,
01:00improving the possibility of effective treatments and interrupting the transmission chain.
01:05Today, research offers treatments such as long-term antiretroviral therapy,
01:10a promising option in the management of HIV, with significant advantages for the quality of life of patients,
01:15because it reduces the number of unnecessary supplies, improving access to treatment.
01:20Improving adherence is a fundamental thing in our field,
01:24as well as the de-stigmatizing aspect of a therapy that is not brought home.
01:29The patient does not have to bring the boxes with him, but he has to take them at certain times.
01:33It is very important, and this has also been emphasized by both the patient associations and the patients themselves.
01:38Currently, the Campania region intends to focus the management of HIV patients within hospital structures,
01:44complicating access to more modern therapeutic options, such as long-acting treatment,
01:49which can be performed only in ambulatory structures,
01:52but to obtain the exemption of the drug, it is necessary to overcome regulatory obstacles.
01:57The Campania region is the only one of the few that still remains with a management of patients
02:02who live with HIV in day hospitals.
02:05If they are managed in day hospitals, they cannot obtain this drug.
02:10The problem is that now we have to, in some way,
02:14generate a transition from hospital management to ambulatory management,
02:21which theoretically could already happen.
02:25What is the obstacle at this moment?
02:27We have to guarantee the privacy of patients,
02:30because a patient who does not see the privacy protected,
02:34is a patient who flees, does not take care of himself, prefers to give up therapy.
02:37Or he can leave.
02:38Or he can leave.
