00:00 Today, in the field of large-cell diffuse lymphomas and refractory relapses,

00:04 which represent 40% of non-oxygen lymphomas,

00:07 which is the most frequent disease in the field of voice and lymphomas,

00:11 CAR-T has been introduced about 4 years ago.

00:15 Today in Italy we have two products for CAR-T from the third line,

00:20 a third product will arrive in 2-3 months,

00:23 and this year we will have two products in the second line.

00:26 CAR-T has radically changed the history of several patients

00:31 in this setting of large-cell diffuse lymphomas and refractory relapses.

00:35 Today, about 40% of these patients are cured,

00:41 and they were patients who were practically lost,

00:44 because patients who do not respond to two lines of conventional chemoimmune therapy

00:49 are patients who do not have further chances.

00:52 So the advent of CAR-T has given us the opportunity to cure this 35-40% of patients.

00:59 And this is a fact not only in the field of the three registrative clinics of these CAR-T products,

01:05 but also in the real life, in the everyday life,

01:09 experiences therefore European, American, and also Italian.

01:15 In Italy we have treated more than 500 patients,

01:19 and we will be invited to the next international conference on lymphomas,

01:23 which will be held in Lugano next June.

01:25 CAR-T is a truly special moment,

01:31 because it leaves aside the discourse of conventional chemotherapy

01:35 and we are talking about an intelligent immunotherapy.

01:40 The lymphocytes are taken from the patient with a classic apheresis,

01:46 which are sent to a laboratory,

01:49 they are engineered, therefore armed, to be effective, active,

01:53 exclusively against the lymphoma's celluloplasts of that patient.

01:59 They return after about 15-20 days,

02:02 they are re-infused as a normal blood transfusion,

02:05 and after that they perform their activities.

02:07 This is incredible, because it is the first example of a specific,

02:12 personalized immunological therapy.

02:15 We are talking about many years of personalized medicine.
